is it okay to add some extra attributes to identify and to make the entities more identifiable. like is it okay to create an attribute called Appointment ID in appointment table. Also, can there be tables without a primary key?

Answer by UC No references to attributes from other tables. You will eventually do that in second part of the project. ER diagrams do not have foreign keys.

This is regarding the foreign keys. You mentioned that we are not supposed to mention the foreign keys in ERD. But if we want to make records unique in a table , we may need to use some foreign keys of other tables . For eg patient ID in the patient...
